Who Can Enroll?
Regular Employees
- Working full-time (30 hours or more each week on a regular basis)
Spouse/Domestic Partner
- Legally married spouse (same or opposite sex);
- State-registered same-sex domestic partner
Children
- You or your spouse’s/state registered domestic partner’s biological children, stepchildren, adopted child or foster child up to age 26 regardless of marital or student status.
- Any children for whom you are required to provide coverage under a Qualified Medical Child Support Order.
- Disabled Child: your unmarried children, step-children or children of your domestic partner of any age, if they are incapable of self-care due to a physical or mental disability.
Required Information
- At enrollment you will need to enter the Social Security Number for all covered dependent(s).
- Documentation of dependent status, such as a birth or marriage certificate, will be required for any newly-added dependent(s).
- Health Care Reform requires Zovio to report this information to the IRS each year to show that you and your dependents have coverage and are not subject to a penalty.
- Your personal information will be securely submitted to the IRS and will remain confidential.
Dependent Eligibility
You’re responsible for making sure your dependents are eligible for coverage and will be required to provide documentation verifying eligibility. If it is determined that you have an ineligible dependent enrolled in the plan, you will be responsible for any incurred claims and coverage will be terminated back to the date of initial coverage.
When to Enroll
Open Enrollment
- Open enrollment happens once per year
New Hire
- You are eligible for benefits on the first of the month following your date of hire
- You must enroll within 30 days of becoming eligible
Rehire
- Following a break in employment of 12 months or less, you are eligible for benefits on your rehire date
How to Enroll
Enroll Online in Workday.
- Go to Hotspot and click on the Workday icon
- Click your Profile icon > Inbox, top right-hand corner
- The Change Benefit Elections task will appear
- Note: if you will be adding new dependents, you must add them in Workday before you can enroll them in coverage
- To move through the enrollment pages, click the Orange “Continue” button
- The final step is to confirm your elections
Changing Benefits
Open Enrollment
During open enrollment you can make changes to your benefits – like choosing a different medical plan or enrolling or re-enrolling in the Flexible Spending Account plans.
Qualifying Events
- You can make changes during the plan year if something in your life changes (and is an event recognized by the IRS), such as:
- Marriage or divorce
- Birth or adoption of a child
- Child reaching age 26
- Death of a spouse or child
- Change in child custody
- Change in coverage election made by your spouse his/her employer’s Open Enrollment period
- You lose coverage under your spouse’s plan
If you experience a qualifying life event, you’ll report this by recording your life event in Workday within 30 days of the date of the event. The election change must be consistent with the event. Benefit election decisions will then remain in force for the remainder of the plan year. You are required to submit proper documentation in Workday when adding a dependent. If it is determined that you have an ineligible dependent enrolled in the plan, you’ll be responsible for any incurred claims and coverage will be terminated back to the original effective date.
When Benefits End
Coverage ends when you no longer meet plan eligibility requirements. If your employment with Zovio ends, your medical, dental and vision coverage end on the last day of that month. Your life and disability coverage, access to Flexible Spending Account funds and all other benefits end on the day your employment ends. In some cases, you may be eligible to continue your medical, dental, vision and health care Flexible Spending Account through COBRA by paying the full cost of coverage plus the administration fee.
